- Virginia law requires 6th and new 7th, 8th, 9th, 10th, and 11th graders to have a current TDAP shot.
- 6th grade schedules to be held if no documentation is provided prior to orientation.
- TDAP shots available for children at age 10 at the Peninsula Health Department (757-594-7410).
- Your child's physician may wait until age 11 to give this required shot; if so, we must have written documentation from your doctor.

Whooping Cough for 6th Grade: Virginia state law requires the Tdap booster be given prior to entry into 6th grade. See your private doctor or the Health Department. In 2004, there were 25,827 reported cases in the U.S. However, since whooping cough is often mistaken for other illnesses, it is estimated that over 1 million people get it every year. Preteens and teenagers are at risk since the protection from whooping cough shots given to babies begins to wear off after 5 to 10 years.

A certificate from a licensed physician stating that the student has been successfully immunized against communicable diseases as required by state law at the time of registration.

Parental Report of Varicella Disease
All of the following must be reported by the parents in describing the child's illness.
Acute onset of the illness
Maculopapulovesicular rash without other apparent cause
Generalized and itchy rash with most lesions on the trunk
Lesions crusted over
If the four criteria are met, the school nurse may document the history of varicella disease in Part II, Section 1, of the MCH-213-G form and initial entry.
If all four criteria are not met or the nurse is uncertain, refer the parent to their prviate provide
Students being admitted for the first time to PCPS primary/elementary school (Pre-K through grade 5), are required to provide a report of a comprehensive physical examination, signed by a licensed physician or nurse practitioner, performed within twelve months of the initial enrollment date.* Students who have never been enrolled in a school before must have their physical exam completed on the Virginia State Form (MCH 213-G). Attach kindergarten out-of-state physicals to the MCH 213-G.
For students transferring into PCPS K-5, a copy of a physical examination in their cumulative record, which meets the above requirements, will be accepted. All Pre-K students must have a physical within 12 months of enrollment date** into PCPS.
*The initial enrollment date is the first date of school attendance in any school system (K through 5).
**Enrollment date in PCPS is the first date of attendance, not registration.
